Социальное бремя рассеянного склероза обусловлено существенным влиянием на качество жизни: наличием инвалидизирующих симптомов, тяжелых сопутствующих соматических и психологических расстройств, необходимостью постоянного приема лекарств и их влиянием на самочувствие. Экономическое бремя болезни связано со значимой финансовой нагрузкой на системы здравоохранения и пациентов, в основном за счет высоких затрат на препараты, изменяющие течение рассеянного склероза (ПИТРС), а также с расходами на госпитализацию и возрастанием всех издержек при усугублении инвалидизации пациентов. Около 70–90% всех расходов приходится на прямые затраты, преимущественно складывающиеся из стоимости ПИТРС, в то время как непрямые затраты (временная нетрудоспособность, стоимость ухода, преждевременная смерть) составляют от 20% до 50% в различных странах. При возникновении обострений расходы возрастают в 2 раза. В настоящее время 80–90% всех издержек приходятся на амбулаторную помощь в связи с применением ПИТРС, не требующую госпитализации, однако расходы на госпитализацию, обусловленные необходимостью лечения пациентов с тяжелыми обострениями, по-прежнему вносят значимый вклад в стоимость терапии.</p></abstract><trans-abstract xml:lang="en"><p>The social burden of multiple sclerosis is determined by its significant impact on quality of life: the presence of disabling symptoms, and severe comorbid somatic and psychological disorders, as well as the need for continuous medication and its impact on well-being. The economic burden of this disease affects both healthcare systems and patients. This can primarily be attributed to the high costs of disease-modifying therapies (DMTs), alongside expenses for hospitalization and the escalation of total costs as patient disability progresses. While direct costs dominated by DMTs, account for approximately 70–90% of total expenditures, indirect costs such as temporary disability, cost of care, and premature death range from 20% to 50% in different countries. Noteworthy is that exacerbations can double treatment costs. Although outpatient care accounts for 80–90% of all costs due to the use of DMTs, inpatient care for severe exacerbations still remains a significant cost driver.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>рассеянный склероз</kwd><kwd>препараты</kwd><kwd>изменяющие течение рассеянного склероза</kwd><kwd>анализ затрат</kwd><kwd>экономическое бремя</kwd><kwd>бремя болезни</kwd></kwd-group><kwd-group xml:lang="en"><kwd>multiple sclerosis</kwd><kwd>disease-modifying therapy</kwd><kwd>costs analysis</kwd><kwd>economic burden</kwd><kwd>disease burden</kwd></kwd-group></article-meta></front><body><sec><title>ВВЕДЕНИЕ / INTRODUCTION</title><p>Многие неврологические заболевания являются причиной инвалидизации и смертности во всем мире. Одной из таких патологий является рассеянный склероз (РС). Это хроническое демиелинизирующее заболевание с множественным очаговым поражением нервной системы, в основе которого лежат процессы аутоиммунного воспаления, что ведет к инвалидизации и снижению качества жизни (КЖ) [<xref ref-type="bibr" rid="cit1">1</xref>].</p><p>Раннее назначение препаратов, изменяющих течение рассеянного склероза (ПИТРС), способно положительно повлиять на частоту обострений заболевания во время лечения, а также уменьшить риск прогрессирования по расширенной шкале оценки степени инвалидизации (англ. Expanded Disability Status Scale, EDSS) в течение полугода [<xref ref-type="bibr" rid="cit2">2</xref>]. За последние 20 лет отмечается рост затрат на лечение РС, наибольший вклад в которые вносит разработка новых, более эффективных и дорогостоящих ПИТРС [<xref ref-type="bibr" rid="cit3">3</xref>].</p><p>Применение ПИТРС позволяет снизить непрямые затраты, связанные с прогрессированием заболевания и инвалидизацией, а также расходы на госпитализацию при обострениях, однако их стоимость является значимым финансовым бременем. В связи с этим возникает потребность в оптимизации назначений ПИТРС, в т.ч. с экономической точки зрения. Имеется достаточно большое количество научных работ, оценивающих затраты на применение ПИТРС, а также расходы, связанные с дальнейшим финансовым обеспечением пациентов в виде выплат за временную нетрудоспособность, инвалидность, затраты, связанные с преждевременной смертностью.</p><p>Цель – оценка имеющихся эпидемиологических данных, в т.ч. о распространенности, заболеваемости и смертности, по РС в различных странах мира и изучение вклада прямых и непрямых затрат в стоимость терапии РС.</p></sec><sec><title>ЭПИДЕМИОЛОГИЯ / EPIDEMIOLOGY</title><p>В настоящее время в мире насчитывается более 2,8 млн человек, страдающих РС. По сравнению с 2013 г. количество пациентов с РС возросло на 30%, отмечается рост заболеваемости в каждом регионе. Распространенность составляет 2,1 случая на 100 тыс. населения в год. В структуре заболеваемости сохраняется преобладание лиц женского пола (соотношение женщин и мужчин составляет 2:1, а в ряде стран достигает 4:1) [<xref ref-type="bibr" rid="cit4">4</xref>].</p><p>В России согласно международной статистике общее количество пациентов с РС составляет более 78 тыс. человек. Средний возраст больных – 32 года, 67% заболевших – женщины [<xref ref-type="bibr" rid="cit5">5</xref>]. По официальной статистике в Российской Федерации, общее количество пациентов варьируется от 52 до 150 тыс. человек, что обусловлено отсутствием единой базы данных по этому заболеванию. Распространенность различается в разных регионах страны, преобладание отмечено в северных и северо-западных регионах [<xref ref-type="bibr" rid="cit6">6</xref>]. По данным исследований, она составляет от 20 до 80 случаев на 100 тыс. человек [<xref ref-type="bibr" rid="cit7">7</xref>].</p><p>В Белоруссии общая заболеваемость РС – 41 случай на 100 тыс. населения [<xref ref-type="bibr" rid="cit8">8</xref>]. По данным крупного международного эпидемиологического исследования, общее число людей, страдающих РС, составляет 5 тыс., распространенность – 50 случаев на 100 тыс. человек [<xref ref-type="bibr" rid="cit5">5</xref>].</p><p>Распространенность РС в США в 2009 г. составляла 350–400 тыс. случаев (58–95 на 100 тыс. населения). Заболеваемость достигает 10,4 случая на 100 тыс. населения в год (3,6 среди женщин и 2,0 среди мужчин) [<xref ref-type="bibr" rid="cit9">9</xref>].</p><p>Распространенность РС в Австралии на 2017 г. достигла уровня 103,7 случая на 100 тыс. населения. Средний возраст пациентов – 41 год, 78% из них – женщины. Среднее количество лет с момента постановки диагноза – 15. Отмечается наличие взаимосвязи с широтой проживания [<xref ref-type="bibr" rid="cit10">10</xref>].</p><p>В Северной Испании распространенность РС – 152 случая на 100 тыс. населения. Среди женщин наблюдаются более высокие показатели, чем среди мужчин (184 против 115 на 100 тыс.). Заболеваемость в период с 2010 по 2015 гг. составила 8 случаев на 100 тыс. населения в год. Среди женщин заболеваемость выше, чем среди мужчин (10 против 6 на 100 тыс.). Среднегодовая смертность составила 0,68% [<xref ref-type="bibr" rid="cit11">11</xref>].</p><p>В Италии проживает 133 тыс. пациентов с РС, распространенность заболевания – 80 случаев на 100 тыс. населения [<xref ref-type="bibr" rid="cit5">5</xref>]. За последние 50 лет показатели продолжали увеличиваться, но они не зависят от широты проживания. Наиболее вероятными гипотезами высокой распространенности РС является генетическая предрасположенность, а также различные факторы окружающей среды [<xref ref-type="bibr" rid="cit12">12</xref>].</p><p>Распространенность РС в Венгрии составляет 101,8 случая на 100 тыс. населения (53,9 и 144,8 на 100 тыс. среди мужчин и женщин соответственно). Ремиттирующим РС (РРС) страдают 68,5% пациентов. Средняя продолжительность болезни составляет 11,73±8,37 года. Соотношение мужчин и женщин – 1:2,925. Заболеваемость достигает 4,44 случая на 100 тыс. населения (среди мужчин – 2,44, среди женщин – 6,25) [<xref ref-type="bibr" rid="cit13">13</xref>].</p><p>В Палестине распространенность РС в 2022 г. составляла 51,9 случая на 100 тыс. населения, смертность от РС достигла 5,08% [<xref ref-type="bibr" rid="cit14">14</xref>].</p><p>Таким образом, показатели распространенности и заболеваемости РС продолжают расти во всех странах мира. В структуре заболевших сохраняется преобладание женщин. Свой вклад в увеличение заболеваемости вносят совершенствование критериев диагностики и доступность диагностических методов для населения, но также отмечаются генетические, экологические и техногенные факторы и повышение количества заболевших.</p></sec><sec><title>СОЦИАЛЬНО-ЭКОНОМИЧЕСКОЕ БРЕМЯ / SOCIO-ECONOMIC BURDEN</title><p>Пациенты с РС испытывают значительную социально-экономическую нагрузку, которая включает непосредственно медицинские затраты и психологические аспекты, связанные с этими тратами. Последние возникают из-за беспокойства по поводу финансов, а также снижения КЖ. Отмечается значимое воздействие социально-экономических факторов на приверженность к терапии [<xref ref-type="bibr" rid="cit15">15</xref>][<xref ref-type="bibr" rid="cit16">16</xref>]. Социально-экономическое положение оказывает влияние на развитие психоэмоциональных расстройств [<xref ref-type="bibr" rid="cit17">17</xref>], прогрессирование заболевания [<xref ref-type="bibr" rid="cit18">18</xref>], нетрудоспособность [19–21].</p></sec><sec><title>Социальные аспекты / Social aspects</title><p>Пациенты с РС имеют сниженное КЖ, связанное со здоровьем. Заболевание влияет на мобильность, повседневную активность, а также самообслуживание. С прогрессированием РС ассоциированы боль, усталость и утомляемость [<xref ref-type="bibr" rid="cit22">22</xref>].</p><p>Весомый вклад в инвалидизацию и прогрессирование РС вносят коморбидные заболевания. Чаще других встречается гипертензия, гиперлипидемия, хроническая обструктивная болезнь легких, а также ишемическая болезнь сердца и эпилепсия. Каждое коморбидное состояние увеличивает оценку по шкале EDSS на 0,18. Более высокий показатель по EDSS отмечен у пациентов мужского пола, старшего возраста и с прогрессирующим типом заболевания. Из коморбидных патологий наибольшее влияние на тяжесть и прогрессирование РС оказывают ишемическая болезнь сердца и эпилепсия [<xref ref-type="bibr" rid="cit23">23</xref>].</p><p>При РС часто наблюдаются следующие симптомы: слабость и усталость, тревога и депрессия, головокружение, астения, головная боль, боль в спине. Это свидетельствует о значительном бремени заболевания для пациента. Также пациенты с РС часто имеют сопутствующие заболевания, такие как ожирение, сахарный диабет 2‑го типа, гипертензия, гиперлипидемия, дефицит витамина D, инфекции мочевыводящих путей [<xref ref-type="bibr" rid="cit24">24</xref>].</p><p>Анализ ряда исследований показал, что КЖ пациентов с РС значительно ухудшается при обострениях и прогрессировании заболевания. Назначение терапии ПИТРС положительно влияло на показатели КЖ [<xref ref-type="bibr" rid="cit25">25</xref>].</p><p>Пациенты с вторично-прогрессирующим РС (ВПРС) имеют более тяжелое бремя болезни в виде множества инвалидизирующих неврологических симптомов, ограничения физической и трудовой активности, необходимости в более частых госпитализациях по сравнению с больными РРС. Отмечено, что пациенты с ВПРС вынуждены прекратить трудовую деятельность в связи с тяжестью заболевания. Также для них характерны более выраженные изменения физического состояния по сравнению с больными РРС [<xref ref-type="bibr" rid="cit26">26</xref>].</p><p>Выявлено, что пациенты сталкиваются со следующими проблемами:</p><p>– сомнения в эффективности лечения [<xref ref-type="bibr" rid="cit25">25</xref>];</p><p>– дискомфорт, связанный с необходимостью инъекций [<xref ref-type="bibr" rid="cit27">27</xref>];</p><p>– желание замены препарата с инъекционной формой применения на другой (пероральный прием, более редкая необходимость инъекций) [<xref ref-type="bibr" rid="cit27">27</xref>];</p><p>– дискомфорт, связанный с побочными эффектами ПИТРС [<xref ref-type="bibr" rid="cit27">27</xref>];</p><p>– появление копинг-стратегий, связанных с лечением (смирение с диагнозом и необходимость внедрения режима лечения в повседневную жизнь позволяют ощутить контроль над болезнью, при отсутствии лечения пациенты беспокоятся по этому поводу) [<xref ref-type="bibr" rid="cit28">28</xref>];</p><p>– негативное влияние необходимости применения ПИТРС и условий их хранения на возможность путешествовать и отдыхать [<xref ref-type="bibr" rid="cit28">28</xref>];</p><p>– низкая приверженность к лечению [<xref ref-type="bibr" rid="cit29">29</xref>].</p></sec><sec><title>Экономические аспекты / Economic aspects</title><p>Систематический обзор исследований в странах с низким и средним уровнями дохода показал, что общие затраты на лечение РС составляют от 463 до 58 616 долл. США (46 тыс. – 5,8 млн руб.). Распределение затрат между прямыми и непрямыми – 89% и 11% соответственно. Отмечено, что общие расходы возрастают в соответствии с тяжестью заболевания. Затраты на лекарственную терапию (ПИТРС), а также на лечение обострений составляют бо́льшую часть всех издержек при более легком течении РС. Однако при нарастании тяжести РС преобладающими становятся прямые немедицинские и непрямые затраты [<xref ref-type="bibr" rid="cit30">30</xref>].</p><p>В 16 странах Европы выполнено обсервационное исследование, которое выявило, что произошел сдвиг в оказании помощи пациентам с РС в виде переноса основного лечения в амбулаторные условия в связи с появлением ПИТРС. В данный момент около 80–90% расходов приходится на амбулаторную помощь. Затраты очень тесно коррелируют с тяжестью заболевания. Средние показатели составили от 22,8 до 57,5 тыс. евро паритета покупательной способности в зависимости от тяжести состояния [<xref ref-type="bibr" rid="cit31">31</xref>].</p><p>Общие затраты на лечение одного пациента в Республике Беларусь составляют около 12,1 тыс. долл. США (990 тыс. руб.), причем по большей части они непрямые. Из данной суммы примерно 1,3% приходится на фармакотерапию, что может быть связано с невключением фармакотерапии в стандарты оказания медицинской помощи. Общие потери валового внутреннего продукта оцениваются в не менее 64% от совокупных затрат [<xref ref-type="bibr" rid="cit8">8</xref>].</p><p>В Польше среднегодовая стоимость лечения РС составила 41 тыс. долл. паритета покупательной способности. Прямые затраты находились в пределах 70% от совокупных. Наибольшие издержки приходятся на фармакотерапию. Только 40% пациентов продолжают оставаться трудоспособными [<xref ref-type="bibr" rid="cit32">32</xref>].</p><p>Общие прямые затраты в Испании составили в среднем 4988,8 евро (499 тыс. руб.), показатели варьировались в зависимости от тяжести состояния и достигали 9345,3 евро (936 тыс. руб.) при обострениях, связанных с вовлечением ствола головного мозга или моторных зон коры головного мозга. Средние прямые затраты возрастали в соответствии с нарастанием нетрудоспособности при обострении и составляли 477,5 евро (47 тыс. руб.) на одного пациента при легкой степени и 1740,2 евро (147 тыс. руб.) на одного пациента при средней или тяжелой степени нетрудоспособности [<xref ref-type="bibr" rid="cit33">33</xref>].</p><p>Общие затраты на одного пациента с РС достигли в среднем около 2,1 млн (±882 тыс.) руб. В России они составили 671 тыс. руб., в Швеции – 3,4 млн руб. Наименьшие общие затраты отмечены в Чехии, Польше, Венгрии, наибольшие – в Швейцарии и Австрии. Уровень непрямых затрат был минимальным в России (150 тыс. руб.), максимальным – в Нидерландах (1,1 млн руб.), в среднем – 700±338 тыс. руб. Непрямые затраты занимают от 20% до 50% общих затрат в разных странах [<xref ref-type="bibr" rid="cit34">34</xref>].</p><p>Затраты на приобретение ПИТРС в расчете на одного пациента в год составили от 240 тыс. руб. в Нидерландах до 970 тыс. руб. в Италии. В России средняя сумма затрат на ПИТРС на одного пациента – 383 тыс. руб. в год. Среднее значение расходов на ПИТРС – 570±237 тыс. руб. в год. Однако в структуре затрат в России отмечается максимальный удельный вес расходов на ПИТРС по сравнению с другими странами (более 80%). Затраты на ПИТРС в целом по странам составляли в среднем 30,2±39,1 млрд руб., в России – 45 млрд руб. Суммарное экономическое бремя – от 8,6 млрд руб. в Венгрии до 390 млрд руб. в Германии, в России – 78,7 млрд руб., в среднем – 97,8 млрд руб. [<xref ref-type="bibr" rid="cit34">34</xref>].</p><p>В Австралии годовые затраты на одного пациента с РС составляют 68 382 долл. (4,1 млн руб.), из которых прямые затраты – 30 346 долл. (1,8 млн руб.), они включают расходы на терапию ПИТРС, диагностические процедуры, консультации врачей и госпитализацию. Расходы тесно взаимосвязаны с тяжестью заболевания [<xref ref-type="bibr" rid="cit10">10</xref>].</p><p>В Японии общие медицинские затраты составляют 787,7 долл. (73 тыс. руб.) на одного пациента в месяц, а при возникновении обострения – 3816,8 долл. (353 тыс. руб.). Они возрастают при наличии обострения и госпитализации. Большая часть затрат приходится на терапию ПИТРС [<xref ref-type="bibr" rid="cit35">35</xref>].</p><p>В 2019 г. было оценено общее экономическое бремя РС в США, которое составляет 85,4 млрд долл. (7,8 трлн руб.). Прямые затраты – 63,3 млрд долл. (5,8 трлн руб.) (74%), непрямые – около 21 млрд долл. (645 млрд руб.) (25%), а также 1,1 млрд долл. (101 млрд руб.) – затраты, которые не были включены в страхование. В среднем медицинские затраты на одного пациента составляют 65,612 долл. (6 млн руб.). Наибольшая часть прямых затрат приходится на рецептурные препараты, применяемые амбулаторно (включая ПИТРС – не менее 89%) – 37,9 млрд долл. (3 трлн руб.). Средние затраты на одного пациента, применяющего ПИТРС, варьируются от 57 202 до 92 719 долл. (5,3–8,5 млн руб.) в зависимости от возраста и пола. Другая значимая часть расходов приходится на применение ПИТРС в условиях стационара (6,7 млрд долл. – 620 млрд руб.), а также инфузионных ПИТРС (5,5 млрд долл. – 509 млрд руб.). Непрямые затраты составляют 21 млрд долл. (645 млрд руб.), из которых 16,8 млрд долл. (1,5 трлн руб.) приходится на пациентов с РС, а оставшаяся часть – на лиц, осуществляющих уход. Значительная часть непрямых затрат (8 млрд долл. – 741 млрд руб., 38%) приходится на преждевременную смерть, а также на нахождение на рабочем месте во время болезни (5,9 млрд долл. (546 млрд руб., 28%) или отсутствие на рабочем месте в связи с болезнью (5,6 млрд долл. – 518 млрд руб., 26%). Общие немедицинские затраты составляют 752 млн долл. (69 млрд руб.), из которых 33% (247 млн долл. – 22,8 млрд руб.) приходится на осуществление оплачиваемого ежедневного ухода, 27% (202 млн долл. – 18,7 млрд руб.) – на приобретение специализированного оборудования. Более 342 млн долл. (31,6 млрд руб.) тратится на лечение пациентов с помощью экспериментальных или нетрадиционных методов [<xref ref-type="bibr" rid="cit36">36</xref>].</p><p>Расходы возрастают при наличии обострений и составляют в среднем 48 700 долл. (4,5 млн руб.). Наибольший вклад в объем затрат среди пациентов без обострений и с умеренным обострением вносит фармакотерапия. В группе больных с тяжелым обострением преобладают прямые медицинские затраты. Средние затраты среди пациентов с обострением были выше (48 700 долл. – 4,5 млн руб.), чем у тех, у кого обострения не было (24 730 долл. – 2,2 млн руб.). С нарастанием тяжести обострения увеличиваются затраты. Высокоэффективное лечение ПИТРС может уменьшить их, влияя на количество обострений [<xref ref-type="bibr" rid="cit37">37</xref>].</p><p>Отмечается, что появление разнообразных ПИТРС вызвало рост цен на препараты. В свою очередь, повышение цен может приводить к уменьшению доступности лекарственных средств для пациентов. Данные механизмы включают ограничения на закупку ПИТРС со стороны страховых компаний, а также ограниченные экономические возможности для покупки лекарств самими пациентами, что вынуждает их использовать препараты с меньшей дозировкой или пропускать прием, а также изменять привычный образ жизни и распределение бюджета в семье [<xref ref-type="bibr" rid="cit38">38</xref>].</p><p>За десятилетний период с 2007 по 2016 гг. произошло увеличение стоимости некоторых ПИТРС не менее чем в 4 раза. Ежегодный ее прирост составил от 9% до 15% в зависимости от препарата [<xref ref-type="bibr" rid="cit39">39</xref>].</p><p>В среднем затраты на терапию первой линии составили от 887 тыс. долл. до 1,3 млн долл. (82,2–120 млн руб.). Окрелизумаб оказался наиболее выгодным с позиций анализа «затраты – эффективность» в качестве препарата первой линии терапии РРС с учетом сниженной цены, но не показал такого же эффекта для ППРС. Затраты на терапию второй линии варьировались в среднем от 559 тыс. долл. до 1,2 млн долл. (51,7–111,1 млн руб.). Алемтузумаб опередил другие препараты второй линии терапии РРС по показателю «затраты – эффективность». Остальные ПИТРС были сравнимы по эффективности с поддерживающей терапией, однако расходы на них были значительными. Окрелизумаб, натализумаб и пегинтерферон бета-1a характеризовались более высокими показателями лет жизни с поправкой на качество, однако их стоимость была выше по сравнению с другими ПИТРС [<xref ref-type="bibr" rid="cit40">40</xref>].</p></sec><sec><title>ЗАКЛЮЧЕНИЕ / CONCLUSION</title><p>РС характеризуется значительным социальным и экономическим бременем не только для самого пациента, но и для общества в целом, включая системы здравоохранения разных стран. Применение ПИТРС оказывает значимое влияние на прогрессирование и обострения заболевания, что находит отражение в уменьшении непрямых затрат, связанных с временной нетрудоспособностью, инвалидностью и уходом, однако является значительным экономическим бременем для систем здравоохранения всего мира в виде прямых затрат на приобретение этих препаратов.</p></sec></body><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Спирин Н.Н., Бойко А.Н., Степанов И.О., Шмидт Т.Е.
