Cost-effectiveness ratio (CER) and budget impact analysis (BIA) were performed. All calculations were made in the MS Excel model. CER compared lenvatinib 24 mg/daily, sorafenib 800 mg/daily and Other chemotherapies mix. BIA compared two scenarios: 1) common practice consisting of sorafenib and Other chemotherapies mix; 2) new practice where lenvatinib is used in 6.25% patients. Costs of drugs, visits to oncologist and hospitalization were calculated both in CER and BIA. Time horizon was 2 years for CER and 5 years for BIA.</p></sec><sec><title>Results</title><p>Results. Lenvatinib is a new highly effective target drug for the monotherapy of the radioiodine refractory differentiated thyroid cancer. Lenvatinib significantly increases progression free survival, thus providing in the proposed model, 1628 additional years of life for patients per 5 years. Additional costs at the same time constitute 5 213 900 000 rub. (670.5 to 1184.3 million rubles per year). Calculated unit lenvatinib efficiency stands at 64.95% less than that of the comparisons targeted drug (sorafenib).</p></sec></abstract><trans-abstract xml:lang="ru"><p>Цель исследования – проведение фармакоэкономического анализа применения препарата ленватиниб (Ленвима®) при распространенном прогрессирующем дифференцированном раке щитовидной железы, рефрактерном к радиоактивному йоду, в Российской Федерации.</p><sec><title>Материалы и методы</title><p>Материалы и методы. Проведены анализ «затраты-эффективность» и анализ влияния на бюджет. Расчеты проводились в модели, построенной в Microsoft Excel. В анализе «затраты-эффективность» сравнивали применение ленватиниба 24 мг/сут., сорафениба 800 мг/сут. и других химиотерапевтических средств. В анализе влияния на бюджет сравнивали сценарий текущей практики, подразумевающий применение сорафениба и других химиотерапевтических средств, с новым сценарием: замещение у 6,25% пациентов терапии препаратом ленватиниб. В обоих видах анализа учитывали расходы на лекарственные препараты, посещения онкологов, затраты на госпитализацию, коррекцию побочных эффектов. Временной горизонт модели в базовом анализе составил два года для анализа «затраты-эффективность» и 5 лет для анализа влияния на бюджет.</p></sec><sec><title>Результаты</title><p>Результаты. Применение препарата ленватиниб, нового эффективного лекарственного средства с управляемым профилем безопасности для монотерапии дифференцированного рака щитовидной железы, рефрактерного к радиоактивному йоду, существенно увеличивает показатель выживаемости без прогрессирования заболевания. Анализ влияния на бюджет показал, что применение препарата ленватиниб обеспечивает в рамках предложенной модели 1 628 дополнительных лет жизни для пациентов за 5 лет применения. Дополнительные затраты при этом составят 5 213,9 млн руб. (от 670,5 до 1 184,3 млн руб. в год).
