Pharmacoepidemiological studies play a key role in optimizing pharmacotherapy for various diseases. In particular, significant progress in the treatment of rheumatoid arthritis (RA) and psoriatic arthritis (PsA) necessitates a detailed analysis of the consumption of disease-modifying antirheumatic drugs. The current therapeutic concept for these conditions focuses on achieving and maintaining longterm remission, which positions methotrexate (MTX) as a first-line agent characterized by high effectiveness, safety and advantageous pharmacoeconomic profiles.</p></sec><sec><title>Objective</title><p>Objective: To investigate the characteristics of MTX use in real clinical practice among patients with RA and PsA during outpatient medical care.</p></sec><sec><title>Material and methods</title><p>Material and methods. The study comprised two parts: a retrospective cross-sectional analysis of MTX consumption dynamics in 2018, 2020, and 2023 via ATC/DDD (Anatomical Therapeutic Chemical classification, ATC; defined daily dose, DDD) methodology, and a longitudinal, noninterventional, pharmacoepidemiological study conducted in 2023 using the medical information system “BARS. Healthcare”.</p></sec><sec><title>Results</title><p>Results. The ATC/DDD analysis demonstrated a significant increase in MTX consumption among patients with RA and PsA: from 302,428.6 to 319,114.3 DDDs per 1,000 patients per year for RA, and from 28,157.1 to 310,771.6 DDDs per 1,000 patients per year for PsA in 2018 and 2023, respectively. The most frequently prescribed dose of MTX was 15 mg/week. The primary therapeutic combination for 55.8% of patients was “MTX + nonsteroidal anti-inflammatory drugs”, with 32.7% of patients also receiving glucocorticoids (GCs) as part of this combination. The study identified a high frequency of prescriptions for proton pump inhibitors, GCs, and cholecalciferol, highlighting the activity of diseases under consideration, potential complications, and the necessity for the prevention of clinically significant drug interactions.</p></sec><sec><title>Conclusions</title><p>Conclusions. The conducted study confirmed that MTX remains the main drug for the treatment of RA and PsA. The ATC/DDD analysis demonstrated a significant increase in MTX consumption in recent years, correlating with its therapeutic effectiveness and accessibility. The high frequency of concomitant prescriptions underscores the complexity of treating RA and PsA and the need for an interdisciplinary approach to ensure safety and efficacy of the therapy.</p></sec></abstract><trans-abstract xml:lang="ru"><sec><title>Актуальность</title><p>Актуальность. Фармакоэпидемиологические исследования играют ключевую роль в оптимизации фармакотерапии различных заболеваний. В частности, значительный прогресс в лечении ревматоидного артрита (РА) и псориатического артрита (ПсА) требует углубленного анализа потребления базисных противовоспалительных препаратов. Современная концепция терапии данных заболеваний основывается на достижении и поддержании длительной ремиссии, в связи с чем метотрексат (МТ) является препаратом первой линии и характеризуется высокой эффективностью, безопасностью и выгодными фармакоэкономическими показателями.</p></sec><sec><title>Цель</title><p>Цель: изучить особенности использования МТ в реальной клинической практике у пациентов с РА и ПсА на амбулаторном этапе оказания медицинской помощи.</p></sec><sec><title>Материал и методы</title><p>Материал и методы. Исследование состояло из двух частей: ретроспективный поперечный анализ динамики потребления МТ в 2018, 2020 и 2023 гг. с помощью методологии ATC/DDD (англ. Anatomical Therapeutic Chemical classification, ATC; defined daily dose, DDD) и лонгитудинальное неинтервенционное фармакоэпидемиологическое исследование, выполненное в 2023 г. с использованием медицинской информационной системы «БАРС. Здравоохранение».</p></sec><sec><title>Результаты</title><p>Результаты. ATC/DDD-анализ показал значительное увеличение потребления МТ среди пациентов с РА и ПсА: с 302 428,6 до 319 114,3 установленных суточных доз на 1000 пациентов в год для РА и с 28 157,1 до 310 771,6 установленных суточных доз на 1000 пациентов в год для ПсА в 2018 и 2023 гг. соответственно. Наиболее часто назначаемая доза МТ составляла 15 мг/нед. Основной терапевтической комбинацией у 55,8% больных была «МТ + нестероидные противовоспалительные препараты», у 32,7% пациентов к данной комбинации добавлялись глюкокортикостероиды (ГКС). Исследование выявило высокую частоту назначения ингибиторов протонной помпы, ГКС и колекальциферола, что акцентирует внимание на активности рассматриваемых заболеваний, возможных осложнениях и необходимости профилактики клинически значимых лекарственных взаимодействий.</p></sec><sec><title>Заключение</title><p>Заключение. Проведенное исследование подтвердило, что МТ остается основным препаратом для лечения РА и ПсА. ATC/DDD-анализ продемонстрировал значимое увеличение потребления МТ за последние годы, что связано с его терапевтической эффективностью и доступностью применения.
